Vaccine Requirements: |

The health and safety of our guests is very important to us. That is why we require proof of
vaccination records for all guests staying at Pet-Ritz.
All vaccines must be given at least 5 days prior to lodging or grooming. All puppies and kittens must
have completed the full series of vaccines including Rabies a minimum of 5 days
prior to visiting us. Proper documentation will be accepted for Titers and/or vaccine
exemptions. |

Bordetella (Required every 6 months)

Feline Guests: |

Feline Distemper

Feline Leukemia OR a negative test within the last year |

**Vaccine not required at this time, but highly recommended. Please consult with
your veterinarian to make the best decision for your and your pet. |

Medications: |
|
Should your pet require the Pet-Ritz staff to administer medications, we request
that you bring the prescription in the original bottle. In addition, please include
your veterinarian’s instructions to guarantee proper administration of medication. |
